This NDIS service is designed to support individuals with disability who are going through significant life stage transitions, such as leaving school, starting or changing jobs, moving out of home, or any other major life change. The service aims to provide tailored support to help the individual navigate these transitions successfully and achieve their goals.

Service Activities:

Planning and Goal Setting: Our team will work with the individual to identify their goals and aspirations for their life stage transition, and help them develop a plan to achieve these goals.
Skills Development: We will provide training and support to help the individual develop the skills they need to succeed in their new environment, such as communication skills, budgeting skills, or job-seeking skills.

Monitoring and Review: Our team will regularly check in with the individual to ensure they are on track to achieve their goals, and make adjustments to their plan as needed.
